Midwest Chapter/MLA
Award for Achievement in Health Information Management

The Executive Board of the Midwest Chapter established the award in 1995. The award was announced with a request for applications at the Annual Meeting in Fort Wayne, Indiana.  The Board proposed that the award encourage health science associations and libraries to creatively respond to the challenges of the evolving profession.

The recipient association or library receives a cash award of $500, which is presented to the recipient's representative during the Annual Meeting of the Midwest Chapter. The award is earmarked for enhancing the programming of the association or library.  The award recipient will be asked to demonstrate or display at the following year's Annual Meeting.

Applications should reflect programming excellence as demonstrated by special projects beyond the normal operational programming of the association or library. The special projects should demonstrate advocacy, leadership, service, technology or innovations that contribute to the advancement of health science librarianship. Evidence of the impact of the project on the association or library must be shown.

The application should contain an overview of the project, detail the quality of the contribution in terms of the impact on the membership of the association or library, show how the project has prompted new responses to challenges, and provide evidence of the benefits to the profession.

The project must have been completed during the twelve months prior to the application deadline and have been conducted for at least twelve months within the proceeding two years.

Ten percent of the applicant's association or library's qualified members must be members of the Midwest Chapter of MLA.

Completed applications should be sent to the above address and should be received no later than August 1, 2001.

Selection is made by a committee following established criteria, voting procedures and submitted for approval to the Chapter's executive board.

The Awards Chair of the Midwest Chapter will notify the award recipient by September 1, 2001.  The award will be presented during the 2001 Annual Meeting in Milwaukee, Wisconsin. The award recipient assumes all costs for attending the meeting at which the award is presented.
